St. Teresa Rehabilitation and Nursing Center, overlooking Derryfield Park in Manchester, offers a comfortable, warm, and inviting environment for short-term patients and longer-term residents. The rooms and common areas of this 51-bed center are all on one level allowing for convenient access to common dining, activity, and outdoor areas.
With the park located across the street, not only are residents provided with a picturesque scene with each change of the season, but they can also watch many of the activities that go on in the park. In addition, the adjacent Trinity High School provides student volunteers who regularly visit and interact with the residents.
Skilled Nursing and Rehabilitation Services
We provide rehabilitation therapy and nursing care for patients after hospitalization. Such "post acute" care is often needed after today's shorter hospital stays and may be appropriate following:
General or cardiac surgery
Joint replacement and orthopedic surgery
Traumatic and brain injuries
We also provide complex nursing care for:
Specialized skin and wound care
Nutritional support and diabetes management
Hospice or "palliative" care is available if curative or restorative care is no longer appropriate and desired for terminally ill individuals. Hospice care focuses on providing comfort and pain relief as well as pastoral support and counseling for patients and family.
Alzheimer's & Dementia Care
People with Alzheimer's or similar type of memory impairment usually benefit from a structured routine and supportive social environment separate from other long-term care residents.
Short-Term Respite Care
We provide short-term respite care when an at-home caregiver is temporarily unavailable or in need of extended personal time away.
Individuals with chronic illness or late-stage Alzheimer's disease are cared for with the goal that they attain their highest functional ability. Individualized care programs provide restorative and rehabilitation services as appropriate to achieve that goal.
Other Professional Services available include:
Pastoral and religious services
Discharge planning consultation and coordination of home care service
Mental health services provided by geriatric psychiatrists, psychologists and psychiatric nurse consultants
Licensed dental, podiatry, optometry, and audiology consultants
Beauty and barber services on site
